Dear Parent/Guardian:

Please complete all forms for your child before they enter for a trial visit or enrollment. It is very important that we receive all the attached forms, most of which are required by state law (M.G.L. c. 76, § 15). Your child will not be able to enter school without these forms. Forms to be completed by Parent/Guardian:

1.     Verification Form (Emergency Form): In case of accidents and other emergencies. Be sure to sign and date the second page. Your child will not be accepted at the Newton-Wellesley Hospital Emergency Room in the event of an emergency, without this signature.
2.     Photo of Child: As required by law.
3.     Photography/Videography Permission Form
4.     Disability/Counseling Form
5.     Student Medical Alert Consent Form (Confidentiality of School Health Records)
6.     (If applicable) Free or Reduced School Lunch Meals Program

Forms to be completed by Parent/Guardian AND Physician:

7.     Medication Permission Form - Prescription Medication: This gives our Nursing staff permission to dispense prescribed medication (signature of parent and physician/licensed prescriber is required for dispensing of any medicine).
8.     Medication Permission Form - Over the Counter Medication: This gives our Nursing staff permission to dispense over the counter medication (signature of parent and physician/licensed prescriber is required for dispensing of any medicine).

Forms to be completed by Physician:

9.     Health Record – Physician/Health Care Providers Examination Form: To be completed by physician. Please have your physician/health care provider include a Physician's Certificate of Immunizations, which is required by law. Your child will not be able to enter the school without these documents.

*If a Physical was completed within the last twelve months, that Physical may be used until the next Physical occurs.

*If your child is scheduled for a physical exam and you cannot submit this form immediately, submit all other forms and send the Health Record – Physician/Health Care Providers Examination Form under separate cover.

Access to Records:  Parents of students and students 18 years of age and older have open access to cumulative files in the administration office. Authorized school personnel from the responsible school system have the same right of access to those records as is provided by regulations with regard to public school student records.

Eligibility: All newly enrolled students must be found eligible for special education and on an approved Individualized Education Program.

Experimentation and Research: The Learning Prep School is not involved in any type of experimentation and research.  If, in the future, the school were to become involved in such experimentation, parents would be notified and a separate release form would be sent requiring their signature before their child would be allowed to participate.

Inclement Weather: School closing announcements will be made over AM radio stations WBZ, WRKO and WBMX FM in addition to television Channels 4, 5 and 7. Our database system will also call parents cell phones to inform of school closing. If inclement weather forces an early dismissal, and you are not at home, your child will be sent to the home of the person you have designated as an alternative.

Liaison: Your child’s assigned counselor will be your contact and liaison for all information regarding Learning Prep School.  Once your child has a schedule with an assigned counselor, you are welcome to contact that individual with any questions or concerns pertaining to your child.

Physical Restraint Policies and Procedures: A copy of the Physical Restraint Policies and Procedures is held in the Main Office area, is available online at learningprep.org and is available upon request.

Release of Information: Learning Prep School will not release information that it has received from any other agency without the expressed, written permission of that agency and the parents.

Request for Documents: Requests for documents to be sent to any organization, program, physician, etc. must be in writing.

School Calendar: A current School Calendar is on website.  Please note the school hours: 7:53 a.m. to 2:23 p.m., Monday through Friday.

Student Information Changes: If any information relating to your child changes during the year (such as address, work or home phone, marital status, or guardianship) please notify us immediately as these changes help us maintain communication you with about your child.
